• Licensed to practice as physical therapist within Iowa • Degree from a bachelor's or master’s program in physical therapy • Two years of experience as a physical therapist (community/home health experience preferred) • Current CPR certification • Driver’s license and an insured, working automobile

The Physical Therapist provides physical therapy services within our patients’ homes in the community. Services are provided according to a written physician's plan of care, adhering to all conditions outlined in the Personnel Service Agreement.
